Consent for physical examination if injured. I agree, if at any time, I make claims against MN-19 Truck Wash and Repair for personal injuries including but not limited to workers compensation claims. I will upon request submit to an examination by a physician of MN-19 choice and expense.
I have been advised and agree that all the information that I have access to while on assignment through MN-19 for is considered MN-19 client’s property, whether or not such information is actually considered a trade secret under applicable law. I hereby agree to keep all client information confidential, and I will not disclose any information to any person expect those persons expressly authorized by MN-19 Truck Wash and Repair.
Any and all discoveries, inventions or literally or other works relating to the the works you perform while on assignment or suggested by matters disclosed in conjunction with your assignment, whether patentable, copyrightable it otherwise subject to registration. You must agree to provide a nondisclosure of each invention, discover, literary or other work and further agree to sgn necessary documents and give MN-19 all other reasonable assistance necessary to perfect and maintain whatever right MN-19 deem appropriate without charge to Mn-19 but without expense to yourself.
Notwithstanding the foregoing, this section shall not apply to an intervention for which no equipment, supplies, facility or trade secret information of Client was used and which was developed entirely on my time unless the invention relates to the business or its clients.
